Ongoing Care

Chronic Condition Care and Monitoring

Regular review and support for long-term conditions, so that your treatment keeps up with your life.

Ongoing Care

What this service is

A chronic condition is a long-term health problem that needs ongoing management. Diabetes, hypertension, asthma, HIV, epilepsy, arthritis and heart conditions are all examples. The goal is not to treat an episode but to keep the condition under control for as long as possible.

Ongoing care means regular review rather than occasional crisis care. We check your readings, your medication and your symptoms, adjust what needs adjusting, and help you build habits that fit your life rather than working against it.

Most people with a chronic condition can live a full and active life when it is well managed. Regular review is what makes that possible.

Questions

Frequently asked questions

If your question is not answered here, phone us on 067 575 2528 — we are happy to talk it through.

What conditions do you monitor?

We monitor a range of long-term conditions, including diabetes, hypertension, heart conditions, asthma, HIV and epilepsy. Please phone us about your specific condition.

How often will I be seen?

It depends on your condition and how stable it is. Many people are seen every three to six months once things have settled, and more often while a treatment is being adjusted.

Can I bring my own readings?

Please do. Home measurements and results from other facilities are genuinely useful, and we will go through them with you rather than dismissing them.

What if I run out of medication?

Phone us before you run out and we will help you avoid a gap in your treatment. Do not stop a medication suddenly without advice.
